- Copy of Birth Certificate
- Copies of two other valid forms of I.D. including Social Security card, Passport Verification, present address (envelope with current postmark date), and the DMV application.
- A clear photo of the person obtaining the permit (this photo will not be returned).
- Notarized parental consent form. This form is necessary for 16 and 17 year old student(s).
